In pursuance to advertisement notification dated 21-12-2020, it is hereby notified that the list of District wise provisionally selected BAMS/ BUMS/ Nursing Candidates along with District wise waiting list for undergoing six (6) months Certificate Programme in Community Health (CPCH) through IGNOU and University of Kashmir for session January 2021, for Kashmir Division is placed as Annexure “A” and list of Division wise provisionally selected BAMS /BUMS /
Nursing Candidates for Counseling and placement in Districts (District Kupwara only) other than their domicile district is
placed as Annexure “B”. For Jammu Division District wise Provisionally selected BAMS/ BUMS/ Nursing Candidates for undergoing six (6 months Certificate Programme in Community Health (CPCH) through IGNOUY/ University of Kashmir for session January 2021 is placed as
Annexure “C” and list of Division wise provisionally selected BAMS/ BUMS/ Nursing Candidates for Counseling and placement in districts other than their domicile district is placed as Annexure

All provisional selected candidates shall bring original documents:

(D.O.B, Domicile Certificate, Degree Certificate & Registration Certificate) for verification
All Provisionally selected candidates shall submit a Fixed Deposit Receipt (FDR) of Rs 60000/- for one (1) year in favour of Financial Advisor & Chief Accounts Officer (FA&CAO) NHM, J&K which shall be released after completion of 6 months inservice period as MLHP at a designated HWC- SC, duly certified by concerned Block Medical Officer and the FDR shall be forfeited in case selected candidate fails to join or leaves the job before compilation of six months All Provisionally selected candidates for undergoing six (6) months Certificate Programme in Community Health (CPCH) for January 2021 session
through IGNOU and University of Kashmir, shall submit an affidavit issued by 1st Class Judicial Magistrate (as per Performa available on official website)
